DA Form 1380 – What It Is, When to Use It, and How to Fill It Out

DA Form 1380 is a commonly referenced but often misunderstood form in the U.S. Army Reserve. If you’ve ever completed EBDL courses, conducted Readiness Management Assemblies (RMAs), or performed duties outside of scheduled Battle Assemblies, chances are you’ve been told to “submit a 1380.”

This guide explains what the DA Form 1380 is, when it’s used, and how to complete it correctly.

📝 What Is DA Form 1380?

DA Form 1380, officially titled “Record of Individual Performance of Reserve Duty,” is used to document non-unit reserve training or non-pay duties. When submitted properly, it ensures you are awarded retirement points—and in some cases, compensation—for qualifying duty outside of traditional drill settings.

🕒 When Do You Use DA Form 1380?

You typically use a 1380 in the following scenarios:

EBDLElectronic Based Distance Learning courses such as JKO, ALMS, or ATRRS-based modules. Used to log points for completed online training.
RST (Rescheduled Training)If you miss a scheduled Battle Assembly and need to make up the time, you’ll use a 1380 to document the make-up duty.
RMA (Readiness Management Assembly)When conducting authorized work (e.g., admin prep, planning) outside drill weekends without pay, but you want retirement points.
AT Extensions / Split DrillsTo document duty before or after Annual Training (AT), or split drill support performed outside your unit schedule.
Instructor Support or Non-Unit TrainingIf supporting training events or courses (e.g., MTTs, PME), or working independently under orders.

🛠️ How to Fill Out DA Form 1380 – Section-by-Section

1. DateDate the form is completed (not the duty date).
2. FromYour name, rank, and last four of SSN. Example: CPT John Smith, XXX-XX-1234.
3. ToYour unit or commander. Example: HHC, 350th CACOM.
4. Organizational Unit and StationYour assigned unit’s full name and location.
5. Period of DutyExact date(s) duty was performed. Ex: 12 May 2025.
6. Nature of DutyBrief but descriptive explanation. Ex: Completed EBDL: JKO Course USA-123.
7. Signature of Individual Performing DutyYour signature and date.
8. Signature of Officer Authorizing DutyYour OIC, supervisor, or someone designated to verify duty. Must be someone authorized to validate points.
9. RemarksAdditional context if needed (e.g., training hours, course completion certificates attached, etc.).

🔁 Submission Tips

  • Always check who is authorized to sign your form (e.g., BN S1, company commander, etc.).
  • For EBDL, attach your course completion certificate as supporting documentation.
  • For RSTs, include the drill period being replaced (e.g., “Making up 3rd MUTA of April BA”).
  • Retain a copy of the signed form for your personal records.
  • Confirm with your unit that the points were uploaded to RPAM (Retirement Points Accounting Management).
